Please send us the appropriate documents that must be filled in by the veterinarian during the travel exam (health/export certificates). Please send us a digital copy of the correct paperwork that needs to be filled out by the veterinarian at clientcare@annexvet.ca THREE (3) business days prior to your appointment.


Please be informed that there is a fee for us to fill out this travel paperwork. For clarification on our fees, please respond to this e-mail or give us a call. If the paperwork is not received on time, this can cause a significant delay in the documentation process.
 


WAIVER & ACKNOWLEDGMENT

I, the undersigned, acknowledge and understand that Annex Animal Hospital provides advisory services and conducts certain tests for pets traveling to other countries. I recognize that each country may have unique and changing requirements for the entry of animals, including but not limited to specific vaccinations, diagnostic tests, treatments, documentation, and timelines.
 

I understand and agree to the following:

 

  • To the best of my knowledge, I have provided accurate and complete information about my pet to the hospital (including microchip numbers, vaccination dates, names, destination details, etc.), and all such information is up to date.
     
  • It is my responsibility, as the pet owner or authorized agent, to research and confirm all current entry requirements for the country to which my pet is traveling, including any countries through which the pet may temporarily transit.
     
  • The hospital will make reasonable efforts to advise me regarding common requirements and to perform requested tests and procedures. However, the hospital cannot guarantee that all requirements of the destination country will be identified, fulfilled, or accepted by authorities.
     
  • The hospital is not responsible for any consequences, including but not limited to denial of entry, quarantine, fines, flight or lodging costs, or any other expenses or damages, resulting from incomplete, incorrect, or missing tests, treatments, or documentation required by the destination country.
     
  • I release and hold harmless the hospital and its affiliated veterinarians and staff from any and all liability related to the failure to comply with any international travel requirements, whether due to changes in regulations, miscommunication, or any other reason.
